Thank you for considering the South Bend Clinic as a community partner for your event and/or organization. Due to the fact that we receive many requests annually, we have listed several guidelines to ensure the request receives proper consideration.
The core purposes of the South Bend Clinic are to provide exceptional patient-centered, physician led, coordinated care to improve the health of our patients and community.
Guidelines for Consideration
Organization should be non-profit
Organization promotes the health and well-being of the community for a variety age groups
Organizations that help define the community in a positive way
Provide partnership opportunities with the South Bend Clinic and organization
Provide an educational component through organization
Organizations who align with the South Bend Clinic mission and strategic priorities
The South Bend Clinic does not provide contributions to:
Individuals seeking assistance for trips, including school sponsored events
Research projects, assessments or special studies
Adult sport teams
Individual School Districts
Review and Approval Process
All organizations must complete the request form in order to be considered. Organizations may submit proposals in addition to the request form. If sending additional information, please make us aware on the submission form.
A maximum of one request per calendar year may be fulfilled per organization.
Donations are NOT automatically renewed or assumed. Each submission is evaluated on a first-time basis and will not be approved based upon a prior year's participation.
Requests are reviewed once a year in the Fall for the upcoming year.
