Patients with Health Insurance

We find that our patient’s insurance can change, so we ask to see your insurance card at every visit. This ensures that you get the maximum value of your benefits and avoids any errors in processing

If you are unable to present evidence of insurance, then you may be held personally financially responsible for payment of all services provided.

  • Copayments are due at the time of service.
  • Payments on your account can be made at any Clinic location or through our website.
  • It is your responsibility to know your insurance benefits, identify any prior authorization requirements, and laboratory or hospital preferences.
  • We submit claims to your insurance carrier for you. You will receive a bill for any patient-due amount
    after insurance processes your claim.
  • Many preventive services or wellness visits are covered in full by your insurance, but we may need to submit a claim and bill you for medical conditions discovered during your exam.
  • We cannot change the doctor’s diagnosis codes to change the outcome of an insurer’s benefit coverage or payment.

Uninsured Patients

  • A payment of $100.00 is requested at check-in for your self-pay services. A discount does not apply if your claim is filed to insurance.
  • We offer a 20% discount for self-pay patients when paid at the time of service. (Excludes: cosmetic services, eyeglasses, hearing aids, immunizations, supplies, and pharmacy).
  • You will be asked to sign a self-pay waiver stating you do not have any active insurance.

Missed Appointments

Please give us the courtesy of 24 hours’ notice should you know you need to cancel or miss your appointment. Without such notice, a fee of $25 or $50 may be charged, depending on the time set aside for your appointment.

That’s why we work to answer all your questions and guide you through the insurance process. If you feel that you cannot afford the healthcare you need, consult with us on the financial options available.

While changes in healthcare are inevitable, you can count on The South Bend Clinic to remain constant in our commitment to provide you and your family the high level of care you have come to expect.

If you have already visited (or plan to) the Health Insurance Marketplace website (www.healthcare.gov), you know there are many plans and options available to ensure healthcare coverage for everyone.

You will want to review your choices carefully and pick what is right for you. In doing so please know we may or may not be a network participant in all of your options.
